INDIVIDUAL CONSULTATIONS:

 

Primary consultation

$500, 60 minutes

The Primary Consultation is the foundation for all other consultations + includes: 

  1. The Nutritional Assessment Questionnaire (NAQ), which consists of over 370 questions assessing many systems and symptoms of your health. You will receive 7+ pages of detailed results from your NAQ.

  2. An Initial Interview Form, a detailed health history + personal goals assessment

  3. 5-Day Food + Mood Log, which allows me to see what your day to day eating looks like and how that relates to your digestion, moods + energy levels.

With your Primary Consultation, you’ll also receive:

  1. Your Personalized Health Plan (PHP), a 15-20 page document with educational pieces about your NAQ and how your body works, including a detailed list of nutritional recommendations (foods to include and exclude), supplements, + lifestyle factors.

  2. A 10-15 page Supplement Chart of custom recommendations that address your primary health concerns + nutritional deficiencies.

  3. A set of Short Term Goals: 4-6 action steps you can implement right away to start changing your diet and lifestyle in order to reach your health goals.

  4. Recipes! I’d love to share with you some of my staple recipes that make eating delicious AND nutritious.

follow-up consultations + Educational appointments

$60, 30 minutes, $120, 60 minutes

These sessions are designed to give you an educational foundation to understand how your body works and how you can support it with optimal nutrition. They give you the opportunity to share any breakthroughs, reports, obstacles, questions, or concerns you may have and give your practitioner the chance to make necessary adjustments and updated recommendations for your continued progress and healing.

re-evaluations

$250, 60 minutes

Includes a second round of the Nutritional Assessment Questionnaire (NAQ) and a new set of Short-Term Goals. Through taking a follow-up NAQ, you can see comparison charts from your initial consultation and your current one. Nothing is more exciting and motivating than seeing the progress you’ve made in your health.

CHECK-INS

$40, 20 minutes

Mini check-in appointments to provide you with ongoing encouragement, support, and direction in your health journey.

 

*You will be invoiced a $25 fee for any missed appointments. If you need to cancel or reschedule an appointment, please do so 24 hours in advance in order to avoid this fee.

*All consultations must be redeemed within three months of purchase date. After three months, purchases will expire.
